Before the visit, fill in what you can

You do not need perfect answers. The goal is to bring the doctor a clearer starting point than "it has been going on for a while" or "I forgot to mention this."

1. Main symptom

  • What is the symptom?
  • Where do you feel it?
  • How would you describe it in plain language?

2. Timing and pattern

  • When did it start?
  • Is it constant, occasional, or getting worse?
  • What time of day is it most noticeable?

3. Triggers and relief

  • What seems to make it worse?
  • What seems to help?
  • Does food, movement, sleep, stress, or medication change it?

4. Medication and context

  • Any recent medication, supplement, or dose changes?
  • Any recent illness, injury, travel, or major life change?
  • Any related test results, photos, readings, or prior diagnoses?

5. Questions to bring

  • What could be causing this?
  • What should I watch for after the visit?
  • When should I call back or seek urgent care?
  • What is the next step if this does not improve?

After the appointment
